What Qualifies a Patient for Skilled Nursing Care?

Searching for skilled care for a loved one can feel overwhelming, especially when it’s your first time and emotions are running high. You’re not alone — many families find themselves unsure of where to start and what qualifies a patient for Skilled Nursing Care. With the right information and support, you can make confident and informed decisions that ensure your loved one receives the care they need.

What is Skilled Nursing Care?

Skilled nursing care refers to medical or therapeutic care provided or supervised by licensed healthcare professionals. It can occur in various settings, including skilled nursing facilities, nursing homes or long-term care centers. Short-term rehabilitation, also known as skilled rehabilitation, is care provided to a patient for a shorter period, typically following a hospital stay, surgery, or illness. Medical professionals design these programs to help patients recover safely and regain as much strength and independence as possible.

Services in skilled nursing care cover a wide range of needs. Medication management ensures prescriptions, whether oral, IV or topical, are handled safely, with providers and pharmacists monitoring for interactions and nurses administering them correctly. Skilled nursing teams also manage specialized care, including wound care, respiratory treatments and insulin administration. Therapy, which is often a core part of recovery, could include physical therapy, occupational therapy and speech therapy. Along with medical care, patients also receive support with daily routines, nutritious meals and opportunities for social interaction.

When Does a Loved One Need Skilled Nursing Care?

Skilled nursing is the ideal choice for individuals who require care that only a licensed healthcare professional can provide. This level of care is often necessary for people with chronic or acute medical conditions that are too complex for a caregiver to manage alone, such as diabetes or Parkinson’s disease. It may also be necessary when prescriptions require regular administration or when specialized equipment, such as a ventilator, is required. Skilled nursing is also suitable for patients recovering from surgery or managing wounds that need consistent, professional care. Hospice is also available.

Typically, a physician’s order is needed to enter skilled nursing care. Your doctor will evaluate your condition and determine which services will best support your recovery. They will outline your care goals, giving the skilled nursing team the guidance they need to create a plan tailored to you. You can find skilled nursing care in a skilled nursing center, a continuing care retirement community (CCRC), a hospital, an assisted living community or a patient’s home.

Does Medicare Cover Skilled Nursing Services?

Medicare Part A can help cover the cost of a skilled nursing center stay, but the coverage is limited to short-term care and rehabilitation, not long-term custodial care.

According to Medicare.gov, you’ll need to meet a few requirements to qualify for coverage:

  • You must enter the skilled nursing center within about 30 days of a hospital stay that lasted at least three days
  • Your care provider must confirm that you need daily skilled care from nursing or therapy staff.
  • The center must be Medicare-certified.
  • Your skilled nursing services must either relate to an ongoing condition treated during your hospital stay or to a new condition that developed while you were already receiving skilled care.
  • You need care to help you recover, maintain your current health, or keep your condition from getting worse.

If you meet these conditions, your costs are as follows:

  • Days 1–20 – full coverage
  • Days 21–100 – a daily copayment of $209.50
  • Days 101 and beyond are your responsibility.

Other Ways to Cover the Cost of Skilled Nursing

Medicaid is a joint federal and state program that helps cover healthcare costs for people with limited income. For seniors, it can also help cover certain long-term care services, including in skilled nursing settings.

Veterans Administration nursing homes provide skilled nursing care for veterans who require additional support. The cost can vary, with copays ranging from $0 to $97 a day, depending on the veteran’s disability rating, income and service history. To qualify for a VA nursing home, veterans must already be enrolled in the VA health care system.

Many Private Insurance plans will cover a short-term stay in a skilled nursing facility when you need it for recovery after a hospital stay due to illness or injury. Coverage varies by plan, so it’s a good idea to review your policy or talk with your insurance provider to understand what your policy covers.

How to Find the Best Skilled Nursing Center

While word-of-mouth reviews are helpful and a great starting point, the best information is factual and objective. A good place to start is The Centers for Medicare & Medicaid Services (CMS). This government rating authority considers several factors to provide an unbiased review of care providers.

To further help you in your search for a superior short- or long-term care provider, AARP recommends scheduling a tour of the health center you’re interested in. Here are a few things to ask both the staff and other residents about:

  • Staff-to-resident ratio
  • Readmission rate for rehabilitation services
  • Cost
  • Services offered
  • Safety precautions
  • Meal options
  • Activities
  • Opportunities for social engagement

The more you know, the better decision you can make for yourself or your loved one.
